A person with a genuine personal objection to being assigned a USI can apply for an exemption to the Student Identifiers Registrar. To apply for an exemption, complete the Commonwealth Statutory Declaration form.
Send it to the Registrar at the following address:

You must include all the information required in the Statutory Declaration, which you must sign before an authorised witness. The list of authorised witnesses is included in the Statutory Declaration form.
You should note that if you do not provide all the information required in the Statutory Declaration form, your application for exemption will be rejected and you will be asked to submit a new complete application if you still want to be considered for an exemption from the USI.
