Third party authority form
A USI account holder can nominate another person or organisation to be their representative. This representative can communicate with us on their behalf to:
- make enquiries
- update personal details on their USI account.
This is called Third Party Authorisation (TPA).
Third party cancellation form
A USI account holder can cancel a TPA who is nominated on their account at any time.
Aboriginal and/or Torres Strait Islander Evidence of Identity form
If you are:
- a student who identifies as Aboriginal and/or Torres Strait Islander
- assisting an Aboriginal and/or Torres Strait Islander student who requires a USI and does not have access to verifiable identification
you can complete and submit our Aboriginal and/or Torres Strait Islander Evidence of Identity (EOI) form.
Incarcerated Student Evidence of Identity Form
If you are assisting a student who is incarcerated and requires a USI, but does not have access to verifiable identification, you can complete and submit an Incarcerated Student EOI form.
Document Verification Service (DVS) override function
We understand some students may not have had their birth registered or may not have access to a verifiable form of identification. In certain circumstances, an authorised provider can use the DVS override function to assist these students to get a USI.
Once approved, providers can then use the DVS override for students who have an alternative photo/date of birth identification document.
To apply for the DVS override function, a provider needs to submit a System Access Request (SAR) form.
