Hi. Welcome to the student video to help you when you have a problem using your form of ID to create a Unique Student Identifier or USI.
I'm going to start from the start however many of you may have already gone through part of this process and come up against a problem when entering details of a form of ID such as your driver's licence. But firstly you'll need to go to the USI website www.usi.gov.au. From the home page select the 'Student Entry' option as we will be creating a USI. Then select the 'Create your USI' option from the student page.
This page is to remind you to have one of the listed forms of ID ready in order to create your USI such as a passport or Medicare card. Select 'Continue'.
You'll be then asked to agree to the terms and conditions. Select 'Next' to continue.
Choose 'Create your USI' option. You'll now need to enter your personal details. Ensure that your name appears exactly as it looks like on your form of ID.
So if you have a middle name printed on your form of ID just make sure you enter that in this form. Next you'll be asked to enter your date of birth and just confirm that date of birth.
Now you'll need to enter your country of birth and town or city you were born in.
Next select your gender.
The contact details section is next. This is the email address, phone number or postal address to where your USI will be sent. I'll use an email address.
And just confirm the email address. Select 'Next' to continue. Confirm your personal details. Select 'Next' to continue. This page displays the form of ID options. Choose the form of ID you're planning on using to create your USI. I'll select 'Driver's licence'. Then choose 'Next' to continue. Select the state that your driver's licence was issued in.
An example driver's licence will be displayed. This is a good chance for you to ensure yours looks the same. Next enter your driver's licence number. Once you've entered your licence number select 'Next' to continue.
If your form of ID does not verify you will receive this message. "We have been unable to verify your identity." This may be because my name is set out differently on my driver's licence. For example "Sally Jones" without the middle name. Or perhaps my driver's licence has been cancelled which is not possible because I'm a great driver.
In either case you may need to go back and change your personal details so they match your form of ID. You can do this by selecting the 'Back' button twice. You will be able to use the same form of ID three times until you're asked to choose another form of ID.
I'll try driver's licence again after amending my personal details.
Because I have exceeded the number of attempts to use that particular form of ID you can only use it three times. I'm now asked to choose a different form of ID.
If you choose to try a different form of ID make sure your name is set out the same as that form of ID because your name may be displayed differently depending on the ID used especially if you have a full name or a nick name present on one of your forms of ID. For example if your name is "Stephanie" on your passport you need to ensure your name is "Stephanie" in the form not a shortened version like "Steph".
Once you've selected your form of ID again enter the document number. You can check on the example at the bottom of the page.
Make sure the document number is correct.
Again you'll be given three attempts to create the USI using this form of ID.
On the third attempt you will be given the other options for forms of ID. If you are still having trouble you can create a help request by selecting the 'Request help' link. This page will ask you to enter your phone number and some notes about the problem you're having.
I'll enter "My ID is not working." Select 'Next' once you've entered your information. You will receive a reference number for your help request beginning with "HRN". Please note this down. Your request will have been sent to the USI office for investigation. The USI office will contact you once a USI has been created.
