Evidence of Identity forms

An Evidence of Identity (EOI) form can be used if verifiable and non-verifiable identity documents are not available.

The EOI forms are only available for students who genuinely do not have access to verifiable or non-verifiable identity documents and are either:

  • a First Nations Australian
  • incarcerated.

The EOI forms can be used to:

The EOI form allows a nominated witness representative ‘one-off’ authority to act on behalf of the student. This is to support students who do not have access to the USI website or are not able to set up student permissions.
