New Evidence of Identity (EOI) forms

We have recently updated our Evidence of Identity (EOI) forms.

To improve usability, the updated EOI forms are available in Microsoft Word or PDF and can either be edited digitally or printed for manual completion. We hope these changes will support timely form completion for training or education providers.

We will no longer accept the old forms from 1 March 2026.
